LEAD PRACTITIONER – MEANINGFUL ACTIVITY

Forum Housing are seeking to appoint a Lead Practitioner – Meaningful Activity – to the recently re-contracted Bridge Project, Birkenhead.

Key tasks include:

  • Working directly with and managing a caseload of service users;
  • Delivering planned programmes of meaningful activity that contribute to the achievement of sustainable outcomes for service users. To promote strong engagement with service users, signposting these where relevant to enable the achievement of personal goals and aspirations;
  • To be the single point of contact for all meaningful activity, developing strong links and relationships with partner agencies.
